Community Stabilization and Transition  (Up to 90 Days)  

This short-term, intensive service is designed for individuals who are experiencing a behavioral or emotional crisis, or who are transitioning from institutional or restrictive settings (such as hospitals, residential treatment facilities, or correctional facilities) into the community. The goal is to provide immediate, coordinated support that promotes stability, addresses urgent needs, and prepares the individual for long-term success in community living.

Service Objectives

  • Stabilize the individual’s emotional, behavioral, and environmental situation.

  • Prevent unnecessary hospitalization, institutional placement, or recurrence of crisis.

  • Support a safe and successful transition into community-based living arrangements.

  • Build skills, routines, and support systems that promote independence and recovery.

Core Service Components

  1. Behavioral Support and Crisis Intervention

    • Immediate, on-site or in-home behavioral support to de-escalate crises.

    • Development and implementation of individualized crisis/safety plans.

    • Ongoing monitoring to reduce triggers and prevent relapse into crisis.

  2. Therapy and Treatment Coordination

    • Collaboration with therapists, case managers, psychiatrists, and other providers.

    • Coordination of mental health and/or substance use disorder treatment services.

    • Support in attending therapy sessions, following treatment plans, and adhering to medication regimens.

  3. Skill Building and Daily Living Support

    • Assistance in establishing healthy daily routines, including hygiene, nutrition, and self-care.

    • Coaching in problem-solving, coping strategies, and emotional regulation.

    • Guidance in managing finances, appointments, and personal responsibilities.

  4. Community Integration

    • Linking individuals to community resources such as peer support groups, employment services, and recreational programs.

    • Supporting participation in positive social activities to reduce isolation.

    • Helping establish a sustainable network of formal and informal supports.

  5. Transition Planning and Follow-Up

    • Creating a comprehensive transition plan to ensure continuity of care after the stabilization period ends.

    • Identifying long-term housing, support services, and treatment providers.

    • Providing follow-up contacts to monitor adjustment and prevent regression.

Service Duration

  • Provided for up to 90 consecutive days based on the individual’s needs, with the intensity of support decreasing as stability and independence improve.

  • May involve daily or multiple weekly visits initially, tapering to periodic check-ins as appropriate.

Expected Outcomes

  • Improved stability in emotional and behavioral functioning.

  • Successful transition from crisis or institutional care into the community.

  • Increased ability to self-manage and access natural supports.

  • Reduced risk of re-hospitalization or repeated crisis episodes.
