Community Living Supports

24/7 in-home care that helps individuals live comfortably and safely within our agency-supported homes. Our dedicated staff assist with:

1. Personal Care (Bathing, Dressing, Grooming)

  • Assistance with daily hygiene routines such as bathing, showering, and oral care, ensuring privacy and dignity.

  • Support with selecting and putting on weather-appropriate clothing.

  • Help with grooming tasks including hair care, shaving, nail care, and skin care.

  • Encouragement and guidance to perform these activities independently when possible.

2. Meal Planning and Preparation

  • Collaborating with the individual to plan balanced meals based on dietary needs, preferences, and cultural considerations.

  • Assisting with grocery shopping, including making lists, selecting items, and managing budgets.

  • Supporting safe food handling, cooking, and portioning.

  • Encouraging participation in meal preparation to build independent living skills.

3. Medication Reminders and Health Monitoring

  • Providing reminders to take medications as prescribed, while promoting self-administration skills.

  • Monitoring for changes in health or behavior and promptly reporting concerns to appropriate contacts.

  • Assisting with health tracking activities, such as logging blood pressure, blood glucose, or weight when needed.

  • Supporting attendance at medical and therapy appointments.

4. Household Tasks and Transportation

  • Assisting with light housekeeping such as cleaning, laundry, and organizing living spaces to ensure a safe and comfortable environment.

  • Supporting yard care or seasonal tasks as appropriate.

  • Providing or arranging transportation for appointments, community events, shopping, and social activities.

  • Encouraging the development of household management skills.

5. Encouraging Participation in Recreational and Wellness Activities

  • Supporting engagement in hobbies, exercise programs, arts, and other recreational opportunities that align with the individual’s interests.

  • Assisting in accessing community resources such as fitness centers, clubs, or volunteer opportunities.

  • Encouraging participation in social gatherings to maintain and build relationships.

  • Promoting emotional well-being through structured wellness activities, mindfulness, or relaxation techniques.
